The following forms are attached for use when submitting any documents relating to your project. Please ensure you complete the form including the CHREC project number and title; a brief summary of what has changed since the previous edition; and comments from the Principal Investigator about how this may impact on the study. Please submit a single original document with form attached. In the case of some complex protocol amendments, further copies may be requested. If in doubt, please telephone Anne Spence on 9508 1376.
- Administrative Amendments (attachment 8) include changes to research project team; dates; contact details; logistics and other issues which raise no ethical concerns.
- Revised Investigator Brochures (attachment 6).
- Protocol Amendments (attachment 7) include amendments to the research method (including inclusion/exclusion criteria); data analysis or dissemination.
