Patient Information - Hospital In The Home

Not everyone enjoys the hospital environment. Many patients would prefer to return to the comfort and familiar surroundings of their own home while receiving care. Cabrini’s Hospital in the Home Unit (HHU) allows some patients to do just that.

Your doctor or nurse may raise the possibility of your admittance to HHU.  An HHU doctor or Nurse Manager will visit you in hospital to discuss your medical treatment and home environment.  If you are willing to be treated at home, and the HHU agrees, you and your carer will be asked to sign a consent form before leaving the hospital for your home.

The HHU staff are on-call 24 hours a day, 7 days a week, so you can rest assured that care is at hand round the clock.  An HHU doctor and nurse will attend you each day, usually for 45-60 minutes.  Visits will be scheduled to meet your care needs.

All medications, dressings and other requirements are locked away in a special equipment case which will remain with you at home for the duration of your care.

Your observations will be recorded and assessed - just as they would be in hospital - and any changes in your condition would receive immediate action.

Should you need help around the home, the HHU can organise home care for you.

Once you are ready to be discharged from the HHU, arrangements will be made for you to see your General Practitioner, or a Specialist, to ensure your continued care.
