Returning home

Before coming to hospital, be sure to make plans for returning home – talk to your family, doctor, case manager or pre-admission nurse about any needs you may have upon leaving hospital.

Matters to consider include shopping, domestic help, personal care, meals, home safety aids and equipment.

Cabrini staff will assist you and your family to plan and arrange care after you leave hospital.

If needed, community based health and support services will be arranged. A care coordination service is available to facilitate complex discharge planning and can be accessed through the nurse manager of your ward.

Patient Transport

It is important to consider your transport to other facilities or returning home when you come into hospital.


Cabrini Malvern discharge process

Patients will be escorted from the ward to the discharge lounge if they are unable to be collected by Cabrini’s 9.30am discharge time. Time spent in the lounge will provide patients the opportunity to:

  • Speak with a nurse and patient assistant should there be any last-minute queries
  • Go through discharge medications with a Cabrini pharmacist
  • Confirm pick up arrangements with the patient’s friend or family member – a phone is available to patients in the lounge
  • Enjoy tea and coffee and light refreshments prior to collection

Please note, there are certain situations where patients may not be appropriate for the discharge lounge such as patients who require stretcher ambulance. These patients will be collected from the ward.

Patients may be collected at the hospital’s main entrance by parking in the three-minute carparks available directly outside the main entrance of the hospital accessible via Coonil Crescent (off Wattletree Road).
