Palliative Homecare

For Cabrini palliative homecare nurse, Caylin Cooke, being able to provide much-needed care for patients in their time of need is an absolute privilege.

Caylin and her team visit 20-25 patients a day – whether it be for symptom management, emotional support or end of life care.

While a large number of visits are to oncology patients, many are to respiratory and cardiac patients.

“Palliative and supportive care is an integral part of Cabrini’s health service and our palliative homecare program is really unique,” she said.

“It’s priceless being able to support patients and their loved ones and provide care in a place of their choosing.

“It’s beautiful to be able to go into patient’s homes and provide that care. It’s a very unique way of nursing.

One patient, Elizabeth Kelly, has been receiving care in her home since January and said it’s been an invaluable asset.

Elizabeth was diagnosed with breast cancer 10 years ago. She recovered and was able to lead an active life until the cancer returned early this year and she was admitted to the Cabrini Palliative and Supportive Care Unit.

“My prognosis is now measured in months not years. This created tears, but was a huge relief to me that we now all understand the situation and can talk about it openly,” Elizabeth said.

“The at-home service has been a major support, not just allowing me to receive care in my own home, but to also begin a number of conversations with my family about my wishes.

“The subsequent care I have had from the palliative care nurses has been exemplary and a comfort to know that is available on a 24-hour basis.

While often seen as a sad job, Caylin said it’s an honour for her and the team to be able to provide the care they do to their patients.

“Lots of people think it’s an incredibly sad job, and it is sad, but it’s also beautiful,” Caylin said.

“It’s such an honour to be able to care for people at that time of their life, in their time of need.”
